Total vs free testosterone test: which should you order online?

For most people ordering without a clinician's instructions, the total testosterone test is the one to buy: MedlinePlus describes it as the test that measures both bound and free testosterone, and HealthLabs lists it at $49.00 against $92 at Personalabs (checked October 2026). A free and total combination ($99.00 at HealthLabs, $119 at Personalabs) is the upgrade when the unbound fraction matters too; the TRT-monitoring panels are a different purchase. Either way the result is a number to talk to a clinician about, not a diagnosis.

What the three tests measure

MedlinePlus lays out the vocabulary better than any store does. "All the testosterone in your blood is either bound to proteins or unbound (free)", and "Free testosterone is more available for your body to use when building bones and muscles." It then names three tests a provider can order: "A total testosterone test , which measures both bound and free testosterone", "A free testosterone test , which measures only the unattached form of testosterone", and "A bioavailable testosterone test , which measures free testosterone and testosterone that's attached to proteins other than SHBG." Of the last it says "This test isn't commonly done", which is why you will rarely find it as a stand-alone consumer product.

The sellers describe the same split in commercial language. HealthLabs' total test "measures all forms of testosterone combined in a single result, making it a reliable first step for identifying potential hormone imbalances", and its page concedes the limit: "While it does not separate free and bound testosterone, it is commonly used for initial screening, baseline measurement, and general hormone evaluation." Its free test "measures the amount of unbound, biologically active testosterone circulating in your bloodstream", and HealthLabs positions it as "especially useful when symptoms are present but total testosterone levels appear normal", which is the seller's framing, not a clinical rule.

Personalabs' free-and-total page gives the cleanest one-sentence definition of the total figure: "Total testosterone measures all testosterone in your blood — both the portion attached to proteins (primarily SHBG and albumin) and the unbound portion." Labcorp OnDemand's comprehensive test adds a detail the others leave out, that its free value is derived rather than directly measured: if the total is above the assay's reporting limit, the page warns, "In this case, free testosterone cannot be calculated accurately."

HealthLabs' struck-through numbers are the company's own list prices, so compare the $49.00, $59.00, $99.00 and $199.00 figures. Labcorp OnDemand's and Personalabs' figures are the prices printed on their pages on the day read. HealthLabs also prints a 110% price guarantee on every testosterone page. Total only: the default first purchase

The case for starting with total testosterone is cost and convention, not accuracy. MedlinePlus lists it first among the three tests; HealthLabs calls it "a reliable first step" and sells it for $49.00; Personalabs sells its men's version for $92 and notes "No fasting required." HealthLabs' page confirms the same: "Fasting is not required for this panel. You can eat and drink normally unless your doctor has given you other instructions."

Timing matters more than fasting. MedlinePlus says of the testosterone test, "Your provider will take a blood sample in the morning, between 7 and 10." Labcorp OnDemand's page agrees: "It is recommended that your blood sample is collected between 7 a.m. and 10 a.m. when testosterone levels are typically highest." Personalabs' total-testosterone page gives the same window, 7–10 AM. Every seller here is a walk-in draw, so you control the hour; the morning window is the one thing worth planning around.

Free and total: when one draw should answer both questions

If the point of ordering is to see the unbound fraction alongside the total, buy the combination rather than two separate tests. HealthLabs' combined test "provides a comprehensive evaluation of testosterone levels by measuring both total testosterone and free (bioavailable) testosterone in the blood" for $99.00, against $59.00 for free alone and $49.00 for total alone, so the pair costs less than the two single tests on its own price list. Personalabs' combination is $119, and its page says the test "measures not just the testosterone that is attached to proteins in your body (which helps control its release) but also free testosterone that is not attached to proteins".

Labcorp OnDemand's Comprehensive Testosterone Test reaches the same place by calculation: it measures the total plus the binding proteins and derives free testosterone, at $119.25 on sale from $159, with results promised 1 day from when the sample reaches the lab. It also carries the biotin caution that applies to many hormone assays: "we recommend that you wait at least 72 hours from your last dose before taking the test."

Panels: a different purchase

A therapy-monitoring panel is not a bigger testosterone test; it is a safety check for people already on treatment, and the sellers say so. HealthLabs' Testosterone Maintenance / Therapy Panel, $199.00, "includes four essential tests that work together to provide a complete picture of therapy impact": the Testosterone Free and Total Test, an "Estradiol, Ultrasensitive Test", a "Complete Blood Count (CBC) with Differential and Platelets", and a "Comprehensive Metabolic Panel (CMP)". Unlike the single tests, it requires fasting: "You should not consume food or beverages other than water for at least 8 hours prior to visiting the lab."

Personalabs sells several men's panels built around total testosterone, including a four-marker hormone profile with FSH, LH and estradiol and an LC/MS-MS total testosterone assay labelled for women and hypogonadal males; prices for those are on its men's health page. Ordering mechanics that apply to every option here

  • A lab draw, not a kit. Personalabs prints "This is not a home test kit. You will need to visit a local lab for testing" on every product page; Labcorp OnDemand's collection method is "In person at a Labcorp location"; HealthLabs lists "Over 4,500 CLIA-Certified Labs".   • The physician order is included. All three sellers include it in the price; none requires your own doctor. Personalabs adds that "A small number of states restrict direct-to-consumer lab testing; contact us to confirm availability in your state."
  • You must be the patient. Labcorp OnDemand states "Test must be taken by purchaser".
  • At-home kits are a separate decision. MedlinePlus's advice applies: "Talk to your provider if you think an at-home test is right for you."
  • Results need a clinician. None of these pages diagnoses anything, and a single morning value is one data point.

Is a free testosterone test more accurate than a total testosterone test?

They measure different things rather than the same thing at different accuracies. MedlinePlus defines total as "both bound and free testosterone" and free as "only the unattached form". Labcorp OnDemand's page also shows that a consumer "free" value is often calculated from the total and binding proteins, which is why it cannot be reported when the total exceeds the assay's limit.

How much does a testosterone test cost without insurance?

MedlinePlus notes that "Though often considered a male sex hormone, testosterone is present in females in smaller amounts." Personalabs' men's catalog includes an LC/MS-MS total testosterone assay it labels for women and hypogonadal males, and Labcorp OnDemand lists a separate total testosterone test for women. A clinician is the right person to say which assay fits.
